$750 lifetime orthodontics per eligible dependent and eligible children up to age 19 years.

“Employee” must sign Optical Claim Form.

Employee reimbursement – statement/receipts must show patients name, date of service and amount paid.

Most local optometrists and optical businesses will bill the Board.

New Employees are eligible for enrollment on hire date; otherwise employees can only enroll or make changes during “Open Enrollment” in April.

Premiums are deducted before taxes.
